General Description:

Provide psychiatric nursing care, i.e., assessment, medication, education and monitoring of medication regimes and supervision of the general health care of Persons .  Provide trainings, education around medications, symptom management and promote health and Wellness amongst all Adult Community Clinical Services programs.   Complete nursing assessments, referrals, and play an integral role within the psychiatric/medical team. 

Essential Job Functions

Direct Care Responsibilities:

  • Assess Person and communicate observations and recommendations to physicians
  • Work in collaboration with the Care Coordination Entity
  • Complete a weekly schedule of meetings and appointments and submit to supervisor weekly
  • Provide and supervise nursing care/general health care
  • Provide back up nursing services when necessary
  • Work in collaboration with the psychiatric medical team- Participate in Medication Clinic, attend medication reviews, complete required documentation for Roger’s Orders and medication authorizations
  • Monitor Persons medications and side effects
  • Coordinate medication services in compliance with MAP regulations
  • Serve as liaison with local physicians, and other health care agencies, i.e., NARH, BMC, VNA, Elder Services, etc.
  • Provide information and educational training to Person , ACCS staff and families
  • Work as a member of the Integrated Team
  • Provide case coordination, consultation, and referral to other community agencies
  • Ensure each Person has all annual physicals (required by DMH), lab work and necessary diagnostic testing; consult with staff regarding health related issues
  • Provide basic and emergency medical care and make appropriate referrals
  • Coordinate medication services with local pharmacies
  • Participate as a member of the interdisciplinary team offering medical and psychiatric concerns
  • Review all referrals and hospital discharges for Persons history, medical needs, risks and coordinate treatment approaches.
  • Coordinate psychiatric appointments with all new enrolled Persons
  • Work collaboratively with other RNs and provide emergency nursing coverage in the event a nurse needs assistance.
  • Provide supervision in response to Persons fluctuating needs
  • Evaluate, train and physically assist Persons activities of daily living (ADL), work, and community activities in group and individual situations while encouraging independence (clinic, residential program, Persons home, community)
  • Provide transportation for Persons if needed
  • Coordinate IM medication clinics and administer medication
  • Monitor lab work and complete all required vitals, weights, heights and blood pressures when needed
  • Work within the GLE teams to establish healthy menu and snack needs
  • Track and review all MOR’s
  • Train staff in MAP, CPR/First Aid and OSHA and any other required trainings

Administrative Responsibilities:

  • Maintain clinical records and complete all required paperwork in a timely manner
  • Provide consultation in the administration of medication
  • Oversee the supervision of medical treatment, providing required training to staff and interpretation of medical orders
  • Coordinate psychiatric schedules ensuring the prioritizing of Persons with immediate needs
  • Review medical related incident reports
  • Participate in all required staff meetings, trainings and required supervision

Qualifications

  • RN nursing with one year of qualifying experience preferred or nursing diploma or an Associate degree in nursing with one year of qualifying experience preferred in a community mental health program
  • Currently licensed as RN in the Commonwealth of Massachusetts
  • Possess and maintain the Healthcare Provider (BLS) level of CPR or obtain within the first 5 months of employment

Skills

  • Sound knowledge of normal personality growth and development
  • Knowledge and experience in crisis intervention
  • Knowledge and skill in the use of psychotropic medications
  • Experience in case coordination:
    1. Ability to objectively assess Persons and advocate for needed treatment
    2. Effectively work with local physicians and their staffs
    3. Oversee implementation of medical recommendations
  • Experience in sharing skills with staff and clients
  • Ability and propensity to work with a diverse population with a range of psychiatric disabilities
  • Ability to interact with Persons in a consistent empowering manner
