Registered Nurse Case Manager - Office-Based Addiction Treatment

Job Summary

Provides patient centered nursing care to individuals with addiction, primarily Opioid Use Disorder and Alcohol Use Disorder, along with co-occurring psychiatric disorders, within the scope of the nursing license. Interacts with the patient from intake, assessment, initiation of treatment, stabilization, and maintenance phases of treatment. Collaborates alongside a multidisciplinary team of professionals including Psychiatrists, Advanced Practice Providers, Clinicians, Recovery Coaches, medical assistants, and clinical support staff to meet the needs and goals of patients.

Essential Job Functions
  • Adheres to all applicable state and federal regulations; ensures compliance with the organization’s policies and procedures, code of conduct, and all applicable regulatory requirements.
  • Provides patient-centered care within the nursing license scope of practice, including the initial assessment and intake, medication initiation, stabilization, and maintenance phases of treatment, assessing the patient’s current needs and risk status and collaborating with the medical team regarding all changes in risk status.
  • Collaborates with psychiatric medical team, clinicians, pharmacies, primary care providers, recovery coaches, and specialty care providers to whom the patient has been referred.
  • Assesses patients who chronically cancel/no show medical appointments, ensuring uninterrupted access to medications while providing education of the importance of maintaining scheduled appointments for continuity of care. 
  • Coordinates between the OBAT provider and the pharmacy by assisting with prescription processing and refills, prior authorizations, and insurance issues.
  • Completes appropriate documentation within 24 hours of patient encounter in the electronic medical records and complies with the scope of practice and federal, state, and departmental policies when sharing and/or documenting patient care.
  • Administers injectable medications.
  • Monitors laboratory values, including urine/oral toxicology screens as well as metabolic, chemistry, and therapeutic drug levels as ordered by medical staff, reporting abnormal values to prescriber and/or supervisor.
  • Completes state Bureau of Substance Addiction Service (BSAS) mandated patient paperwork at assigned intervals.
  • Collects urine and/or oral toxicology samples, reading and interpreting results.
  • Participates in training and technical assistance activities through Boston Medical Center, Relias or continuing education training as assigned by the (OBAT) Nurse Program Manager. Trainings and activities are required to enhance nursing skills.
  • Performs (BSAS) site review with (OBAT) Nurse Program Manager at least annually to meet (BSAS) licensure requirement. Reports are then sent to (BSAS) for review.

Qualifications, Experience, and Education

  • High School Diploma, bachelor’s degree preferred.
  • Registered Nurse with current MA license.
  • Basic Life Support (BLS) for Healthcare Provider level of Cardiopulmonary Resuscitation (CPR) or the ability to obtain within first 5 months of employment. 
  • Certified Addiction Registered Nurse (CARN) certification or the ability to obtain within three years of hire date.
  • Two years of substance use/mental health nursing experience preferred.
  • Strong computer skills including word processing and spreadsheet software.
  • Ability to maintain (CARN) certification current while employed.
