Essential Job Functions
I. Direct Care:
- Case management of assigned caseload
- Coordination and collaboration with youths, families, residential treatment providers, clinicians and other providers regarding treatment plan development, strategies for intervention and implementation
- Implement treatment plan and treatment strategies
- Outreach to child/family/collateral/clinical/residential contacts that comprise the youth’s team, i.e. school, court, home, residential programs and other community agencies, etc.
- Encourage and assist youths with enhancing natural supports and developing community connections
- Supervise youths in after school programs, extra curricular and other community based activities as needed
- Assist youths in attending treatment and services at the Brien Center and other relevant agencies as stated in youth’s treatment plan
- Participate in treatment meetings for youths on assigned caseload
- Advocate for youths and youth’s voice and choice
- Provide transportation to family visits, meetings, appointments, activities, etc. to facilitate implementation of treatment plan
II. Administrative Duties:
- Complete all paperwork in a timely manner
- Participate in all relevant staff meetings, supervision and required trainings
- Update supervisor on collateral contacts on a regular basis
- Complete all tasks assigned which impact the organized functioning of the program
Other Responsibilities
- Provide atmosphere conducive to enhancing mental health and recovery in keeping with human rights of youth
- Represent the agency in a professional manner at all times
- Report any incident(s) regarding accidents, injuries and unusual events to program supervisor
- Flexibly respond to program scheduling needs and assist other programs when assigned
- Availability required for on-call rotation
- Provide transportation when necessary to facilitate treatment plan
- Other duties as assigned by supervisor
Qualifications
- High School or equivalency required; Bachelor’s in human services-related field strongly preferred
- 2 years experience working in case-management and/or with children/adolescents and families required
- Knowledge of WRAP curriculum preferred
- CPR, First Aid and MAP Certified, or obtain CPR, First Aid and MAP certification within first 3 months of employment
Skills
- Knowledge of community resources
- Working knowledge of computers

- Able to respond flexibly to program scheduling needs
- Support and maintain the principles and policies of the Brien Center
- Maintain ethical and professional standards and boundaries
- Represent the agency in a professional manner in all community contacts
- Demonstrate commitment to the Agency’s mission and community mental health principles
- Valid drivers license and ability to pass Driver’s License check
- Ability to pass Criminal Offense Record Inquiry (CORI) and DEEC Background Check
- Able to use personal vehicle for business use
Working Conditions
- Works in office; temperature regulated
- Facilitate groups in other community locations
Physical Conditions
- Manual & visual dexterity; correctable.
- Able to work or travel inside and outside as needed in varying weather conditions
- Able to travel and work in various types of community, residential and other settings
- Push, pull, stand, reach overhead and twist
- Able to bend at the knees, waist and lower self to the ground or floor without assistance
- Able to physically climb and descend stairs without assistance
